Q: BSOD Bugcode USB Driver

I've been dealing with an intermittent BSOD problem since I got my new laptop. I had to have a tech person come to my house twice already to replace a bad screen (and motherboard the second time). My only option now would be to ship the computer to Dell for repair, and I would prefer to avoid that if at all possible. I've attached the requested files in the FAQ thread.

The error I get on my BSOD is Bugcode USB Driver, and it happens whether I have anything plugged into the USB ports or not. It happens at completely random times. Troubleshooting steps I have tried: Updated all drivers I could find, uninstalled and reinstalled USB. I'm running Home Premium 64bit, OEM, original install, and my computer is about 4 months old.

If anyone has any suggestions, I would really appreciate hearing them!

Might be as simple as your Avast.

I notice your avast which is often at least a contributing cause of BSOD'S . Please remove and replace your Avast with Microsoft Security Essentials AT LEAST TO TEST

Hello, you called it correctly, usb issue. Go to device manager , you can see it in Control Panel, open it up and update the Hub Usb driver. Your mini dump showed it has failed.

I've been getting this BSOD at least twice a day for the last 6 months

It's starting to drive me mad, i've tried reinstalling the USB drivers and disabling the USB ports which made no difference. The screen also turns red when it is moved, i'm not sure if this is related to the problem.

I would first remove AVG including Identity Protection. AVG is a known cause of BSOD and same as third-party anti-virus. Use this removal when done to clean up the left overs: AVG - Download tools

Use Microsoft Security Essentials (MSE) as replacement: http://www.microsoft.com/security_essentials/

Remove CD virtualization software such as Daemon Tools/Alcohol 120%. They use a file sptd.sys, found in your dump which is notorious for causing BSOD. Use this SPTD uninstaller when done: DuplexSecure - Downloads
Select Uninstall, DO NOT SELECT RE-INSTALL.

Greetings all. I NEED HELP! This has been driving me crazy for months now. It seems I get a Bugcode USB BSOD error intermittently. Sometimes it happens after I open the lid on my laptop (not in sleep mode or hibernating.) Sometimes it just happens out of the blue (sorry, no pun intended.)

Hopefully I have attached everything, please let me know if you need more.

In simple terms, I struggle to switch my laptop on. Generally it takes around 15 minutes to get to desktop going via a blue screen and windows 8 auto repair on start up. Key points:

- Never boots straight to desktop. First screen is always a choice of repair and refresh options in booting up. All of which apparently fail.

- Sometimes it takes a few repairs and restarts but generally around 15 minutes before it launches into the desktop which is no guarantee that it doesn't shut down once it arrives.

- USB BUGCODE ERROR is given by the crash report.

- Once all is working the laptop will have no real problems and can be used fully until you switch it on for the next use.

- Same issue happened in Windows 8, but Windows 8.1 seems to have taken the 'repair' stage a bit longer.


Any help would be much appreciated as this is very annoying, as 15 minutes is an average for booting to a successful desktop.

Probably caused by : usbehci.sys ( usbehci!EHCI_sMode_PollEndpointSlot+68 )
The bugcheck indicates that the host controller wasn't using the correct physical address allocated by the USB driver.

I believe the fourth parameter is a pointer to the USB_ENDPOINT_DESCRIPTOR data structure, which doesn't seem to display at all within WinDbg. The !devext extension isn't supported anymore for USB buses. I'm not entirely sure if the USB_CONTROLLER_INFORMATION_0 data structure is the correct one, but it seems to also be having symbol problems.

The third parameter is the PCI Vendor ID code, and is split into two parts: 8086 (VenID) and 1e26 (DevID), the Vendor ID points to Intel. Using the DevID and checking your MSINFO32 file, we can see that the driver belonging to your Intel(R) 7 Series/C216 Chipset Family USB Enhanced Host Controller, is causing problems.

Hello and thanks for taking intrest and wanting to help with my problem. I'll begin with a desription of how the error takes place. when I attempt to shut down my computer from time to time it will pop up a blue screen error thats reads usb bugcode driver but thatis all I can read for the sreen goes by to fast. after this my computer restarts its self instead of shutting down... my computer is less than 30 days old and i dont know if this has sometihing to do with it but i have a moto droid that when i attached to my computer installed a driver for transferring data and another for tethering could these be making problems and if so can i see them for they can be removed.
if anyone who reads this could help me it would be much appreciated im not extremely advanced in computers but i know the basic so the more simple insrtuctions may work best for me. thanks for your time.

First, Unfortunately, the Windows Debugging Tools aren't able to access symbols for your operating system files (in particular your Microsoft USB drivers) from the Microsoft Symbol Server - so that makes debugging them difficult if not impossible.

Please do the following (this will not affect the analysis of the current files - but it may help later files that are acquired):
- activate/validate the Windows installation at Genuine Microsoft Software
- run sfc.exe /scannow to replace any problem files
- open a support incident with Microsoft to see if they can fix the missing symbols issue ( Windows 7 Solution Center )
- If that doesn't fix it, then wipe the hard drive and reinstall Windows
- don't use "leaked"/torrent builds

Because of the above issues, there's not much info in the dump files. But there is one suspect driver and it's associated with a phone (qcfilterhp.sys Tue Mar 18 20:11:27 2008)

I would remove the phone software and ensure that the above driver isn't loading.
Then see if you have further BSOD's.

So I have had this BSOD alot, just recently it has began to mess up my laptop. Last night January 31, 2013, I had to do a system restore because it wouldn't start properly. I have also had the polizei canada virus and had to do a system restore than too. So anyway I need to know how to fix this because it is still getting the BSOD. Hi,

My laptop has been randomly crashing lately with a BSOD, Bugcode USB Driver. I'm running windows 7 home premium 64 bit.

I've also noticed a message randomly pop up in the taskbar saying something about a USB device has stopped working, even though there are no USB devices connected to my laptop at all. Also, if I adjust the angle of the laptop screen, I sometimes get the 'ba dum' sound windows makes when a device has been plugged in or out. I am thinking that the integrated webcam is somehow connecting and disconnecting when I move the screen. That may be completely unrelated, but am mentioning it just in case.

Hi,
So I have been have some issues for a while now and have not been able to do much on my own to solve them. Problem seems to occur when my wireless adapter (TL-WN722N) randomly stops working resulting in:
1. Try to reboot - hangs on windows shut down, eventually a BSOD
2. Try to disable/re-enable in Device Manager. Device Manager stops responding. See #1
3. Try to uninstall/reinstall in Device Manager. Process hangs, eventual BSOD.

I have tried old and new drivers with the same results. I have also tried disabling the power saving feature in Device Manager.

The second issue is on bootup, randomly hangs at windows startup. I have to reboot from there and then the RAID controller indicates my RAID needs to be verified. Once into Windows intel rapid storage verifies the RAID and all seems fine. Usually if I shut down prior to the adapter failing everything ends up fine.

Try updating your mouse and keyboard drivers as well as any software associated with them.

Run an sfc/scannow command in an elevated command prompt. If errors are found, run it twice more with each scan followed by a reboot.

You have AVG installed, is that correct? At least to test, uninstall it and install MSE (Microsoft Security Essentials) instead.

These drivers can be found by searching them in Google and then finding the associated software or driver. Finally, find the latest version of this driver or software and download and install it. Some of the drivers may also be found here (if the driver says 'Windows Update' simply make sure all Windows Updates are installed).

You should also update any other drivers you can think of including graphics, audio, chipset, SSD etc.

Make sure all Windows updates are installed.

Hi, i've had this Asus for about 2 months now and i've experienced 3 BSOD's. They are random and i almost kinda think they happen if i leave the laptop on for a long time, like say; a couple days, but i'm not quite certain.

The issue is USBHUB.sys. Thats an OS file that is timing out waiting for a USB device to respond.

Some other things of note

Related to atkwmiacpi64.sys ATK WMIACPI Utility from ASUS. I would remove it.

So the title pretty much sums up my problem. It started a few weeks ago where I get that error message whenever shutting down or restarting my comp. I find that if I uninstall the USB drivers I can restart with no problems and then the drivers are re-installed. However, when I shutdown again I have the same problems.

Also, to make things more fun, when the computer attempts to shutdown it takes a really long time and when it finally restarts I get another BSOD with a different message.

I've included some dump files so I appreciate any information.

Hi,

I've encountered the dreaded bluescreen three times this week.

- It always occurs whenever I attempt to shut down.
- I attempted system restore.
- I've done my research with this bluescreen/bugcode usb driver and one of the things I tried was to go into device manager and uninstall universal serial bus controllers. I did this and the windows update would automatically reinstall them?
- I uninstalled SP2 and all the Microsoft Visual C... I now have 3.5 SP1 and KB2467175/KB2467174.
- I've used Ccleaner and Hijackthis to delete/repair whatever registry bugs there were.

The only time this error and bsod comes up is when I try to put music on my Sirius Stiletto 2. The whole bottom numbers are 0x000000FE(0x00000005 0x82E1B0E0 0x11063038 0xFF81DF70). Any clue on how to fix this. Sorry for the lack of sense, been up for 22 hours on 2 hours of sleep.

I guess you install chipset drivers or any other system device drivers which could have overwritten the USB 2.0 drivers. So reinstall the USB 2.0 drivers and it should get solved.

Hello
I recently downgraded to windows 7 64 bit from windows 8 - a fresh install and when the PC comes back from the sleep mode it freezes for about 15 seconds and then reboots with the Bugcode usb driver blue screen error. I noticed in device manager its missing 2 drivers (SM Bus Controller and Universal Serial Bus (USB) Controller) and I cannot seem to find them anywhere can someone help please? My ISP's Talk-Talk, but they're calling this 'problem solved', so I'm hoping you can tell me which way to go on this one (and probably what to do when I get there....!!)

Did have a Speedtouch 330 modem but it stopped synching. MicroScope told me my computer was fine, reinstalled the Speedtouch drivers, no joy. So they gave me a nice, new, FAST Huawei EchoLife HG532. Now this works like a dream as long as I'm not doing anything that makes it 'busy', by which I mean if I'm just browsing text sites like this, or E-mails, no probs. If I start doing anything like streaming, I keep getting BSODS saying Bugcode USB. I haven't put the number because that changes, I'll append it if it comes in useful as an error range check or similar but it's different every time.

Now I've read that it could be a driver issue but I've checked Event Viewer and all seems happy there. And I've looked in USB ports under Device Manager and none of them have little yellow question marks or exclamation marks. I've got the beastie plugged into a powered hub but I'm STILL wondering if it's leeching too much power from the system when it's 'busy' - streaming video or streaming audio. That's the time it's most likely to hit the Bugcode BSOD - when it's streaming.

I seem to have a bug in the USB drivers or such that I need help with analyzing. I have tried deleting the USB drivers and letting the system update the driver. Good afternoon. I'm using a Dell Latitude D630 with a built-in Dell(Broadcom) 1395 Mini Card being used as its WLAN Adapter. The card is worthless - has no Capture whatsoever. I've attempted installing a Realtek RTL 8188 CUS module that has a 3db antenna hooked to it. I'm currently using a W8.1 Single Language Build 9600 OS . When the Realtek module goes to connect to the Xfinity WIFI target, I get a BSOD Error Bugcode NDIS device. If I even begin to believe that the W8.1 won't recognize the RTL 8188 driver because it's obsolete and I'll need to use the NDISwrapper tool I once used with Linux, I'm gonna scream and continue until my vocal chords are toast. And, if I will need to disconnect the Broadcom Card because of the conflict between two WLAN's on one Network, I WON'T do it and accept defeat. I've already totally disassembled this D630 and it was simply the hardest, most time consuming refurbishment I"ve done in my life. I'm a CET and won't let Dell defeat my purpose. I've learned along the way that you fine folks are the last word on W8.1 Bugs. And, with that in mind, I'll be standing by and patiently await your feedback on how to bypass this impending grief. What on earth is this about? I got a blue screen of death when watching a DVD a few minutes ago which gave an error code of wording of USB BUGCODE

Once i restarted the computer it took me to the microsoft site, and says "An unknown error occurred in ADSL USB Modem (glausb.sys)."

MSN error message analysis

Whilst I have chronic broadband connection problems which no-one wants to fix (my line is 5 miles long the furthest they can work here) I've never had this type least of all, when not even being connected to the net and playing a DVD instead.

I can really see AOL being helpful with this or BT who provides the (useless)broadband connection so wondered if anyone had any ideas about this?

If it is a rare error you can probably ignore it. But if it is repeating or if you are having modem or connectivity problems generally -- you should look for updated drivers for your USB DSL modem. Perhaps even just removing and reinstalling them would help.

If there is no joy in seeking a solution this way, you might look into getting an ethernet modem instead of a USB one

I have just finished removing a virus with the help of the virus forum and was referred to this forum for this ither problem, the computer restarts about 5 to 10 times before it will stay running and lately now and then i get a blue screen that is dumping physical memory to disc, here is the info from event log in device manager event viewer/system/save dump

The bugcheck does not tell me anything.
Please turn off "Automatic Rebooting" and you will get a blue screen error message. This should give us a bit more information

Right-click My Computer / Properties / Advanced / Startup and Recovery/Settings.
Under System Failure, uncheck the ?Automatically restart? check box
You will then get a BSOD with error message which will help identify the problem.

Please post the message such as
STOP 0x00000000A IRQL NOT LESS OR EQUAL
and any drivers named ( *****.sys)

You could also try disconnecting all devices and see if it helps.

I have had BSOD for a while and being super busy I did not have time to look at it and get it fixed. Well Being a computer guy I assumed I could fix it myself. But with tons of research and trying to use every guide I can find I decided to try here(probably should have came here first). Today I started Driver Verifier and having it check all drivers. It crashed a couple times before I realized it was a process I had to stop. I then came here to try and get some help sorting out the driver. (I also have an Ubuntu partition.)

I'd left my mp3 player plugged in overnight + an Asus Xonar DAC in a usb hub, when Windows wanted to do an update and conflict over drivers ensued. Strange, because I've selected for notification before updating. Also, have not been successful installing any new updates since the end of May. Was seemingly a common problem, I checked into it, too busy/lazy to pursue and succeed. This part is confusing...?

Woke up to blasting fans and unplugged everything peripheral. First, tried setting back to restore points prior to that night - twice. Could restart in Safe Mode and with networking. Then, could restart normally - but, shutdowns after 20 minutes or so.

Got crash logs, saw driver conflicts (not found) - saved.

Came here, read threads, did restart at Last Known Good Configuration. Restart - Nvidia Geforce driver not found. Went to Asus site, downloaded and installed driver (not lastest version, but last approved version and not much later than one that was installed).

It then went into a system cycled start/restart/etc. attempting to find driver, fix itself - forced to do shutdown to stop it.

Restarted this morning in Safe Mode, followed your instructions for posting. It's been running in safe mode for about 20 minutes now.

Hello,

I would appreciate any help with the following problem: the pc sometimes seemingly randomly shows BSOD with errors named in the title, I also attach corresponding screenshots. The problem wont happen if the PC power is supplied from the adapter AND there is some content running on the screen (video in VLC f.ex.). Other important symptoms are: Can not turn off the laptop. There will be endless blue screen saying shutting down,while PC already stopped working. I also can not get the PC to sleep. If i I go to sleep mode, display turns black, PC is still working on and on and I can not go back to desktop anymore.

Hello,

After many many hours of searching forums for solutions I have been unable to find a solution to my problem - so I decided to open a topic of my own and hope someone can help me with my problem specifically.

My problem is similar to this topic, although the solutions posted there aren't working for me. In short: I have been experiencing random but frequent BSODs (16 and counting) with the message 'driver power state overload' being displayed.

In my quest for a solution I wanted to upgrade my drivers, when I noticed my Nvidia Geforce 650M graphics card seems to have disappeared. When I try to install the latest driver from Nvidia's website I get the error 'graphics driver could not find compatible graphics hardware'. Attempting to open the Nvidia control panel gives me the message 'An Nvidia graphics card was not detected on your system' and I cannot find the card listed in the device manager (only the intel HD graphics family is listed under display adapters)... To be clear: I'm quite sure my laptop SHOULD have this card, it's just not detecting it for some reason.

Since the symptoms of my problem are exactly the same as described in the topic linked above, I have tried to follow the solution described by Arc. Installed Driver Fusion, started in in safe mode and tried to delete Nvidia drivers - but nothing seemed to be listed here. I still clicked delete but not sure if this actually did something or not.

I am receiving random BSODs with the subject line description.  My first stop was to the HP Support site to possibly update any drivers (like bios??).  I found many driver updates available, including 2 for the bios.  I really have no clue as to which driver updates to install, even after reading the details.  I looked for where it says recommended or required or something but didn't see it.  I then installed the HP Support Solutions and did a scan to determine what drivers it thought I needed and it came up with none.  Now I'm real confused.  Does that mean all the driver updates on the HP site are just optional?  Can someone provide a good site link to read about how to determine which drivers are needed?  Thank you for your help!

Same error again... I was using Google Earth, and my external monitor went black (this happens when I'm out of RAM, but I have additional 4 GB paging file, and 16 GB physical RAM), I turned my laptop to sleep (usually I can fix this black screen problem by turning to sleep and back on) and back on, and it showed BSOD - SYSTEM_SERVICE_EXCEPTION:

BTW I don't have those crash-causing NVIDIA drivers installed for 2 months now, all I have is Intel HD 4600 Graphics which I installed in non-traditional way (by using 'Have Disk'), because there is no other way, Toshiba doesn't provide drivers like these anymore, so I had to update using original Intel drivers (they are compatible with my processor and intel graphics version).

I just ran driver verifier, and here are 3 BSOD messages that showed up

Hi, I've recently been using drivermax to update my drivers. After my updates, I've been randomly getting BSODs then and now. I looked into the minidump with bluescreenviewer but cannot find a fix to the problem. I've uploaded the minidump. Please help! I'm using a shared a computer so I don't know if the origin of the problem is caused by someone else.

Hi all.

My computer has always had intermittent BSOD but generally it's been only 1-2 a month and after a system restart everything works fine. In the last couple of weeks I've been getting BSOD after only a few minutes of operation every time I start up and generally after these errors the computer won't boot - sometimes it will be a black screen after BIOS check, sometimes it won't even get that far - just black screen without the BIOS check even starting. Usually the only way to get it to boot again is to disconnect the power and remove the CMOS battery from the motherboard. I've run driver verifier which has indeed generated it's own minidumps but I don't really understand what these error reports are trying to tell me - if someone could take a look and point me in the right direction it would be much appreciated.

Your BIOS dates from 2010. Please check at the manufacturer's website to see if there are any BIOS updates available for your system. Also check the readme file to see if they apply to the problems that you are having. Please be advised that flashing the BIOS is a very unforgiving process - and one mistake can physically damage your motherboard.

Only 128 Windows Updates installed. Most systems have 160 or more. Please visit Windows Update and get ALL available updates (it may take several trips to get them all).

I recently started a new job and was given a new Dell Laptop. I am having seemingly randomly BSOD issues. Some days it doesn't happen at all and some days it can happen up to ten times. I've got several BSOD errors (mainly CRITICAL PROCESS DIED). I believe it's a driver

related issue. I've tried updating all my drivers and firmware to no avail.

I used the Driver Verifier and immediately got errors with sedfilter.sys and symefasi64.sys. I had to boot into safe mode to turn the verifier off. I tried the verifier without those two enabled and got DRIVER VERIFIER DETECTED VIOLATION.

Help would be greatly appreciated, this issue is a real pain!

I have a relatively new computer that I've built and I've been getting BSOD's for a while.

At first I thought it was because the pagefile was disabled, but that was just why they weren't getting logged to MiniDump. Then I got it enabled again and now I'm getting a lot of BSODs. It's been 0x3b ntoskrnl exe lately, but when I enabled driver verifier I was getting a different BSOD on each boot.

One of them specifically mentioned a .sys that I can't exactly remember. it was XXhalp.sys or something, and when I Googled it - everyone was talking about deleting monitor profiles from the registry and that somehow old monitor profiles were causing the BSODs, so I donno about that.

I would appreciate any help you can offer. I'll be actively monitoring this (and I'm currently in safe mode with driver verifier still enabled). So just let me know what you need.

I get a BSOD only when gaming Heroes of Newerth which really only uses a small portion of my computers power, other games I have not noticed any issues (although I dont game other games much). Movies and general computering does not cause any BSOD.

I did a Driver Verifier BSOD came up hidclass.sys saved.. Arghh I had to do a system restore when I came back to windows and it deleted the mini dump! I've attached a old ones and will re-do driver verifier if requested.

I did a windows ram test and HDD test for C: and no issues noticed.

I'm new to this whole process but tried to figure it out on my own first any help is much appreciated!

A:BSOD while gaming - Driver Verifier makes BSOD hidclass.sys

I've tried to set up a Driver Verifier mini dump but when I activate it it makes a bluescreen when I come back. I follow the instructions to then go back to my system restore point but when I do it then removes the dump file. My assumption is that now I want for another natural BSOD and then upload that dump? Or should I redo the Driver Verifier and in safe mode upload the dump files?

Hi. I've been having this problem for about one year now and I decided it's time I sought online help. My PC is about 2 and a half years old and the current windows installation is roughly 2 years old (windows 7 professional x64 from microsoft academic alliance...).
Crashes/freezes happen at random times (I had gone about a month with none) and not when a particular program is running. I've attached minidump files and system health report.

I keep having various BSODs on boot only, and after some hours of searching, it seems to come down to memory addressing out of range with drivers, according to the web. However, I could be wrong, and that is why I'm here. NTFS.sys, kernel errors, and IRQL_NOT_LESS_OR_EQUAL, 0x0000007E, seem to be the popular ones that have occurred and all multiple times over the last few months. I've nuked all the drivers and updated all the ones I can think of from chipset to peripherals. I'm tired, and not as experienced as the people here with debugging, so I figured I'd dump my report and ask for help. What is causing this boot freeze-up? Also, it doesn't happen every time. I can restart the computer up to 3 times max, and once in windows it is fine no matter how long I use it, or what hardware I use. ZIP is attached. Thanks!

I would suggest you to uninstall Avast using Avast Uninstall Utility. At least as a test. Use Microsoft Security Essentials as your antivirus with windows inbuilt firewall, and free MBAM as the on demand scanner.

I've tried formatting windows 8 and also tried installing drivers seperately for both cards. Nothing has worked so far for me. I am trying to run these cards in crossfire. Both are 7950. As soon as it gets to the part where it installs the display driver, the screen goes black the first time, shortly thereafter the screen goes black second time and suddenly I get a bsod with atikmdag.sys. I'm running:

Windows 7 Home Premium, 64-bit, OEM, never reinstalled
HP model #p6654y, approx. 1 year old, no major problems until now

I've attached the BSOD Dump and System File Collection, but Windows refuses to produce a System Health Report because I have to run in safe mode. If you have an alternative method, I'll be glad to try it, but normal mode seems to be producing crashes more and more quickly.

First BSOD was 12/25, then 12/29, then 12/31. There have been more than 3 crashes, but not every crash has produced a minidump; several times, the crash has been a sudden black screen. One time, display froze with a bunch of horizontal lines/distortion and stuck like that until I hard restarted. I have had to hard restart every time (except for 2 occasions during which hard restart would not work, and I had to pull the power cord).

System Restore had me up and running for a whole day yesterday, and then it crashed again today. I was playing Zynga Poker on Facebook during at least 2 crashes, but crashes have happened without it open as well.

Running 2 passes of the memory checker did not detect any memory issues. From my experience, it doesn't seem like a hardware problem, but I could be wrong.

Please help.

That crash often indicates issues with graphics card drivers or the hardware itself and is rarely problems with DirectX (though it can be a problem with Windows corruption).
DirectX comes installed with Windows, so this may indicate Windows corruption. It may also be that you have corrupted drivers or a graphics card hardware problem.
If you are overclocking any hardware, please stop.

Check Windows for corruption. Run SFC /SCANNOW Command - System File Checker up to three times to fix all errors with a restart in between each. Post back if it continues to show errors after a fourth run or if the first run comes back with no integrity violations. Use OPTION THREE of SFC /SCANNOW Command - System File Checker to provide us with the sfcdetails.txt file if errors occur.

Recently my computer has started seriously acting up by first disabling all directx and d3d functionality it took a full driver uninstall and reinstall to make that functional again but my computer is still having bsod issue's I don't understand where this came from it happened out of nowhere my 6950 is also overheating for some reason also out of the blue its pushing 103-105C in directx11 games which it usually never hit over 60-70C in on ultra settings which is the new temp the card idles at with the case open and fans pointed at it. I've been having a problem with plugging in a USB Wireless Mouse. I had put one into the 1 of my three USB ports. 2 on the left, one on the right. All other USB ports now are not recognizing this piece of hardware and crash upon them being put in after 30 seconds or so. Anything other than these two mouses works perfectly. The USB ports do not recognize either of my mouse wireless receivers and will cause the laptop to crash upon being put inside any of the ports. Things such as my Laptop Cooling Pad and the jacks/USB for my Turtle Beach X12 headset which I'm using both of these right now without them crashing my laptop. I have also tried deleting the USB Root Hub items in the Device Manager via Control Panel, however on attempt of deleting two certain ones (probably related to my mouses hardware), I get stuck on the 'Uninstalling' screen after right clicking USB Root Hub and 'Uninstall'. After 30 seconds it crashes to a BSOD containing the information posted below in my Minidump file. If anyone has enough time to read AND find a proper solution, it'd mean my entire life because this computer, I can't afford to lose it or anything else inside of it. It has very valuable information on it and if I lose it, I'm done for. So if anyone could help, I'd really appreciate it. Additional problem provided below:

Yesterday i popped in my motherboard cd and installed asus suite II and reinstalled the Renesas usb 3.0 drivers, Updated my BIOS to a more recent version and raised my ram speed from 1333 to 1866mhz. A short while after this my computer BSOD and it said PFN_LIST_CORRUPT. I used seagate tools to check my hard drive for problems, it came back clear. Chkdsk didnt fix anything. Right now im making a IMAGE of my hard drive and are going to use seagate tools to fix any corrupted sectors. after this PFN_LIST_CORRPUT bsod a few hours later another bsod came on saying "A driver attempting to corrupt the system has been caught. The faulty driver currently on the kernel stack must be replaced with a working version." This second BSOD was caused by the driver verifier i activated, so i shut off driver verifier to prevent it from making any future BSODS. I have been experiencing the BSOD recently, every week or two. I traced the problem to a usb driver, though I am so new to Windows 7 that I am not sure how to track down which one is causing the problem. The BSOD message mentions etronc.sys, assuming I have the spelling correct on the file name.

I have a home built computer, with a gigabyte motherboard, GA-A75-D3H, and an amd cpu, AMD A6-3500 APU with Radeon(tm) HD Graphics, 2100 Mhz, 3 Core(s), 3 Logical Processor(s). 4 GB of ram, Windows 7 home premium, 6.1.7601 Service Pack 1 Build 7601. Upgraded from Windows XP just before Thanksgiving 2011. 500GB SATA drive. The culprit is supposed to be the Etron USB 3.0 Extensible Host Controller, IRQ 4294967294.

Any help would be appreciated. Thanks.

P.S. I think my motherboard is about 2-3 years old. Bought the cpu and memory along with the motherboard. The DVD writer is only about 1-2 months old.

I forgot to mention that my HP photosmart 5515 printer goes into sleep mode within a few minutes, so that may be why the status html says it is disabled. Also, I am using microsoft security essentials as my antivirus program. Not sure why that was not identified as such.

I don't know why, my brand new system is kinda failing, after i installed the Microsoft security essential, some programs began to crash and even a bsod occured, i had the memtext ran with 3 passes, everything is brand new..

I believe the problem may be the F540 headset, since it don't have proper windows drivers, the only way i got it to work is by pluggin both usb and aux in in my pc, otherwise, if i just plug the usb port, the audio controller will show it turning off and on (disappering and appearing) repetiadly.. on the device manager, there's a driver with a yellow mark, don't know what driver it is, the system doesn't say anything.
Tried to update every driver on the system with the latest ones from aSUS website, and also provided a ez flash bios update

Getting BSoD after aswRvrt.sys loads (even trying safe mode). I read around that it has to do with either Avast, a Windows update, or both and most responses to this issue has to do with FRST. So I got it to run with recovery console finally, and it created an FRST.txt

So Ive had this problem and figured out it was my USB 3 driver. Under device manager I have two USB host controllers and two USB Hub. When installing the drivers it only looks like 1 set installs correctly and then I get the windows error "the driver did not install correctly" I have unistalled and re-installed the latest driver several times including as administrator. I ended up disabling 1 of the 2 USB (the one that had the mark on it in device manager) and that seemed to work. I was going to wait until a new driver was released but I got another BSOD today (been about 10 days with no BSOD) I havent checked for the past 2 weeks on an updated driver but if there are any thoughts id like to hear them. I think my next option is to just uninstall the driver completely until a new driver comes out.

I was trying to get my canon multi-funtion to work as a scanner and clicked on a Canon IJ network to try to get the appropriate driver for it. The computer went to crash dump and i was only able to get it to start in safe mode. I disabled the Canon IJ in startups via MSCONFIG, that did not fix it.

At one point it was doing a crash dump and freezing at 100% dump and I had to use the power button to restart. I removed two programs with the Canon IJ scanner name and the crash dump now goes all the way to restart. In normal mode the BSOD comes with the starting of the logon page. In safe mode with networking it lets me plug in my password before showing me the BSOD.

I do not have the windows debugging tools installed and cant do it now with out networking, unless I try downloading another computer first to install on the offending computer. What else should I try?

My laptop, a lenovo ideapad y510p, has had a couple bluescreens lately after a history of no crashes and bugs. The error message in them is DRIVER_POWER_STATE_FAILURE.
This is the error information in event viewer:

My HP Pavillion dv6 is only around 2 months old

I've removed Daemon Tools and Hotspotshield,which I've heard can cause some problems.Reinstalled the drivers from AMD but BSOD still appears.

The BSOD always occurs when I'm using chrome and I have Youtube videos open in IE,which is the only browser I can watch Youtube videos from(also a problem)

I have the same card but in a dv7. I ran memtest and all that. it seemed stable under 11.5b drivers, then i updated to 11.6 and the notebook was just freezing, then i upgrading to 11.7, the driver crashed a lot. now i have just got that bsod and the same driver culprit. so i have rolled back to 11.5b. ***.

Hi all,

Weird one... I had a couple of USB devices plugged in, with HDMI and power. I pulled them all out and received a BSOD... maybe a usb driver?

Difficult to say, as you have supplied a single crash dump only (not any other necessary files) and that crash dump is kinda silent.

Test your RAM modules for possible errors.
How to Test and Diagnose RAM Issues with Memtest86+
Run memtest for at least 8 passes, preferably overnight.

If it start showing errors/red lines, stop testing. A single error is enough to determine that something is going bad there.

If memtest86+ results no errors, enable Driver Verifier to monitor the drivers.
Driver Verifier - Enable and Disable
Run Driver Verifier for 24 hours or the occurrence of the next crash, whichever is earlier.

Why Driver Verifier:
It puts a stress on the drivers, ans so it makes the unstable drivers crash. Hopefully the driver that crashes is recorded in the memory dump.

How Can we know that DV is enabled:
It will make the system bit of slow, laggy.



   Warning
Before enabling DV, make it sure that you have earlier System restore points made in your computer. You can check it easily by using CCleaner looking at Tools > System Restore.

If there is no points, make a System Restore Point manually before enabling DV.



   Tip

If you fail to get on the Desktop because of DV, Boot into Advanced Boot Options > Safe mode. Disable DV there. Now boot normally again, and try following the instruction of enabling DV again.

Hi, teakiie, and welcome to the forum. I looked at your four most recent dumps. They all have different error codes and they all point to memory corruption.

I recommend that while you are troubleshooting, that you uninstall Comodo. I would download Revo Unistaller and use it to remove Comodo. Though Comodo generally runs well with Win 7, it is known to cause BSOD's on some Win 7 systems. Download and install Microsoft Security Essential and make sure Windows Firewall is turned on. After you get your system stable, you can try Comodo again.

You have an almost guaranteed BSOD maker on your system, Daemon Tools. It uses a driver, found in your dmp, sptd.sys, that is notorious for causing BSODs. Use this SPTD uninstaller when you're done: DuplexSecure - Downloads. Select uninstall; do NOT select reinstall.

Here is a free alternative CD virtualization program that does not use the sptd.sys driver.

I have an asus u52f bbl5. I get random shutdowns with an irql not less or equal error. I have updated the driver to the latest version. I have also completely uninstalled the WiFi card and reinstalled. The shutdowns happen random as far as I can tell. I ran blue screen view and it pointed to the WiFi driver. Do I have a bad WiFi card or is this software related? The card is an Intel WiFi 1000.

My Nvidia 8800 gts has the latest drivers (8.17.12.5721) (Vista 64 bit) - I am wondering if they could be the cause, but i don't have the previuous drivers to roll back to.

Any ideas how to decipher the code. I have latest Windowspatches and have not installed any new hardware. I did reset the BIOS recently as system would not boot (power would only come on for a second).

Strangely i seem to lose some settings like desktop resolution and web pages dont remember my passwords after the crashes.

I have an HP 32 bit desktop running Vista Home Premium SP2. Was running fine until about 1 month or 2 ago when it started getting BSOD (Memory_Management, Page_Fault, etc). I have run through the hardware diagnostics and all passes. Turned on verifier and saw Driver error but screen blanked b/f I could see which driver. Turned on verifier with >verifier /volatile /all /Log <Log>. After the computer BSOD'ed again I checked log file but could not identify any errors (Crashed b/f writing driver error to log?). Any ideas on how to find the offending driver? I don't think this machine came with a Vista disk. Has an E: recovery partition.

Upon installing a VIA HD Audio Driver for my M4A87TD/USB3 motherboard (http://www.asus.com/product.aspx?p_i...aaa&templete=2 under downloads, Windows 7 x64, audio), my computer BSODs and tells me its due to "a driver mismanaging PTEs." I've tested and I'm sure this is the responsible driver.

I've system restored and tried installing again but the results were the same. Without the driver my 5.1 speakers only function as 2.0 speakers.

This driver has worked with me before on a previous install on the same computer. It also works on another identical computer in the flat.

So I went and acquired ATI's newest driver,version 12.3 for my ATI HD 5870 and attempted to install it, and thats pretty much as far as I got. The software installed bits and pieces of AMD extras that come with it (CCC, Hydravision, etc.) and then as soon as it stared installing the driver, windows came up with the BSOD. I have only been able to run my card on v8.6... and nothing else.

I would like to update it now as some of the fixes that come with the new driver will actually benefit me.

Just thinking of something completely different, would NOT updating windows 7 cause an error like this to occur?? and are there any important updates that may help?? (Im running out if ideas as you can tell... )

We do need the DMP file as it contains the only record of the sequence of events leading up to the crash, what drivers were loaded, and what was responsible.

If you are overclocking STOP
We could also use some system information, which you can get easily by running msinfo32.
To do that go to start>run>type msinfo32>enter

When it is finished running go to file>save>name it and upload to us here.
You may be able to get the DMP files without crashing by booting into safe mode (F8) with networking.

To enable us to assist you with your computer's BSOD symptoms, upload the contents of your "\Windows\Minidump" folder.

I've been getting a lot of BSOD's lately, this happened after newly installing a GPU, Mobo, PSU, and CPU.
I'm not good at this debugging, so I was wondering, could I upload the files somewere for someone to help me figure it out?
This is really frustrating, because there all new parts at one time, so I can't really tell, but the reason I think its the GPU is because, lately I have been getting a lot of "Driver stopped responding and recovered successfully, from my GPU.

Hello all

Hi - I have started getting BSOD when running apps.

BSOD Suggests driver issue. The error messge is:

DRIVER_IRQL_NOT_LESS_OR_EQUAL

also mentions uguru at the end of message (motherboard is abit ip35pro)

I have uninstalled uguru now and still get the error.

My Nvidia 8800 gts has the latest drivers (8.17.12.5721) (Vista 64 bit) - I am wondering if they could be the cause, but i don't have the previuous drivers to roll back to.

Any ideas how to decipher the code. I have latest Windows patches and have not installed any new hardware. I did reset the BIOS recently as system would not boot (power would only come on for a second).

Strangely i seem to lose some settings like desktop resolution and web pages dont remember my passwords after the crashes.

The memory.dmp file (converted to word) has this in it [I have cut stuff out that repeats] - again mentions uguru.

I have been getting random BSODs (9F-driver power) when sleeping my new HP Elitebook 8540W. The previous one indicated a file assoc with the wireless card was the issue so I went to Intel site and loaded their newest version - I previously had HPs latest version.

That seemed like it was going to work till yesterday when I got another. This one is indicated as ntoskrnl.exe+70740. Same 9F code. Previous ones (except for the above) were also ntoskrnl. This info was obtained via bluescreenview. Maybe they are to separate issues.

This is W7 x64 ultimate which was a clean install on a new HDD. It came with XP downgrade. XP loaded with W7 x86 Pro disk.
The system is 6 months old with a new MB because of an audio/bluetooth failure.
ALL HP the latest HP drivers are installed except for the one for the Intel pro 6300 wireless.
I do not have the fingerprint reader driver installed yet -
CPU = i7-620, F880 graphics, intel 6300 wireless, broadcom bluetooth,

HP support can only say it is a ACHI issue.

how can it nail this down?

Hey guys
I very recently bought a laptop from a firm called novatech. Everything was fine untill i tryed to synch my HTC desire phone with my laptop when i had no internet signal. Since then it has bluescreened baisically everyday since. I sent it back to Novatech and they told me it was a busted hard drive and replaced it. When it finally returned it crashed with exactly the same problem! I have reinstalled windows and still no luck!

I followed JCGriff's instructions (although the perfmon /report gave me an error). Here are the zip files requested for the BSOD. been plagueing me for over a week now. Everytime I seem to get things working I get the recurring BSOD and can only use the machine in safe mode....

It's hard to say whether software of hardware related, but I lean more toward software at this time because of many out-dated drivers. They are clashing with your very up-to-date Vista SP2 x86 system. You have brand new NVIDIA video drivers (2010), yet have apps that interface with the 2010 drivers that were written back in 2004 - Sentry Works screen capture software, for example.

I have two problems:

I can't open msinfo32; when I try I get the windows help and error window. I posted on the XP forums before being referred here:

Did you run the suggested scans? Adaware and Spybot S&D? I can see evidence of Housecall's ActiveX control...did it find anything?

There is no malware showing in your HJT log. From your other posts, it seems like you have windows issues. That said, we can use another scanner to see if there is any malware present. Please do the following:

Please download Ad-aware at http://www.lavasoftusa.com/ and install it if you don't have it already. Make sure it's the newest version and check for any updates before running it. Also go to http://www.lavasoftusa.com/software/...2cleaner.shtml to download the plug-in for fixing VX2 variants. To run this tool, go into Ad-aware->Add-ons and select VX2 Cleaner. Then click Run Tool and OK to start it. If it's clean, it will say Status System Clean. Otherwise, you will have to click on the Clean button to remove the VX2 infection. Recently I have been having numerous (15+ Times a day) Blue Screens of Death detailing "DRIVER_IRQL_NOT_LESS_OR_EQUAL" (I will post the dump log further down)

This has never happened before and I can not remember installing any new applications, Windows updates, or making any changes at all to my system prior to them happening. I must say, it is very random how it occurs.

Just some history before I get to the technical parts. I have ran Registry Mechanic thoroughly on my system and have had no luck, AVG and Nod32 have found no viruses or otherwise malicious coding on my machine, and Ad-aware hasn't found anything too critical either.

My system is as follows:
Windows Vista Ultimate 32-bit (Last update installed 8/30/07: KB938979)
Gigabyte GA-965P-DS3 (rev 3.3) motherboard
Intel Core 2 Duo Extreme X6800 @ 2.93 GHz

* After working on this problem for the past few days I believe the issue is occuring with the driver for my NIC (The Marvell Yukon 88E8056)

Unfortunately due to the BSOD occurring randomly it is difficult to reproduce easily, as I have not found exactly what triggers it but I have tried updating the Driver to a Vista 32 bit version with no luck as well all the other drivers I have used have not fixed the problem.

A:Driver IRQ issue and BSOD

The file in the dump log "yk60x86.sys" seems to be playing a part in the crash and after some research this is a file used in the drivers for the Marvell Yukon 88E8056 PCI-E Gigabit Ethernet Controller.

Furthermore, this crash really only seems to occur while the device is enabled.

I checked the IRQ resources on my machine, and noticed that this device, my video card, and a few others are all using IRQ 16 which might have something to do with it, I'm not too sure, but my system will not allow me to change the IRQ assigned to the device.

Hi

Yesterday i installed Alchohol 120 on my PC and when it rebooted it has been BSOD since. It wont let me enter Safe mode (or variants) or start Last Known Configuration, Repair options wont work and only place i can do anything seems to be in Repair Advanced. From there i can launch a DOS window and can manipulate some Windows.

If i try a System Restore i obtain "Unspecified Error" and aborts.

I dont wish to reinstall Windows and lose my C drive as my files are still there and this has only happened as a result of the install that has gone wrong. The System Restore shows Alcohol 120 was installed and files exist in Program Files.

I have an activity log from setupapi.dev.log that shows this install yesterday but i do not know how i may uninstall this software/driver from my limited position.

Can anyone help please?

I just installed a Gigabyte G1 970 GPU in my computer, replacing a 6970 GPU. Before installing the new drivers, I uninstalled the ATI drivers via ATI's uninstaller followed by a reboot. After installing the GeForce drivers, I am getting occasional BSOD's when closing games (Half Life 2: Episode 2 in this case). I also just returned from running errands and noticed my computer had crashed while I was out.

This started as soon as I installed a new GPU and therefor I believe it's probably purely driver based.

Attached are my SF Diagnostic files including the minidumps.

Hi all,

I've been getting BSOD's randomly on my computer. I've had a look at the minidumps to see if they would give a straightforward answer - how wrong I was! I've still got them so I can attach them here if they're needed.

I've had a look on the net at what the most common causes are, so I've removed any software that I've recently installed, other than essential stuff (only Outlook that I can think of).

The BSOD's are still happening though; they used to happen whenever I tried running a certain piece of software, so I uninstalled that and then reinstalled it. That didn't make a difference.

I also updated my graphics driver as I read that they could cause them. That hasn't made a difference either.

Does anyone know what can cause these? I'll see if I can get back on the computer and post the minidump in a bit...
